Speak Up incident reporting

We want you to understand what we will do with the personal information that you give us in this Incident Reporting Form. Personal information is information about a person that can identify that person.

Why we are collecting personal information

We are collecting personal information so that we can appropriately manage incidents, including seeking further information about the incident as part of an investigation, offering support, and incident reporting.

You can choose not to provide your name and contact details, or choose to provide a fake name. However, please be aware that if you do not provide your personal information then we may not be able to properly investigate an incident and take appropriate action.

What information we collect

We may collect details such as full name, contact information, address, date of birth, gender, cultural identity, health and ability information, living circumstances, and main language. The amount of information we collect depends on whether you are reporting an incident or you are the person affected by the incident.

We may collect your information directly from you, or from someone else if they are reporting an incident on your behalf or you are otherwise involved in an incident. If we have your consent, we may also collect your information from your health provider if it is relevant to an incident.

Who we share your personal information with

We may need to share your personal information to keep people safe or to follow the law, which may include reporting an incident to relevant government agencies.  We may also share personal information with emergency services such as police or ambulance, identified health professionals such as your doctor, and your emergency contacts. For some types of incidents, we may need to share your information with our insurance provider or external lawyers or auditors. If you are worried about this, you can speak to us.

We may also share personal information with third parties that provide services to us, such as our IT provider for this incident system if it is necessary so that they can provide services to us (Protecht.ERM Pty Ltd (ABN 50 158 878 347)).
